A relationship-first outreach and navigation model.

P.A.V.E uses a model designed to reach individuals who are often disconnected from traditional systems of care. Our work focuses on trust, consistency, navigation, warm handoffs, and follow-through.

STEP 01
Outreach

We meet individuals where they are — on the street, in encampment areas, in shelters, in high-need communities, and through partner referrals.

STEP 02
Engagement

We build trust through consistency, lived experience, compassion, and trauma-informed communication. We remain patient, respectful, and persistent.

STEP 03
Assessment

We identify immediate needs and barriers related to substance use, housing, mental health, medical care, re-entry, safety, identification documents, transportation, and family or community support.

STEP 04
Navigation

We help individuals understand their options and connect to services that fit their needs — treatment referrals, MAT, housing, behavioral health, community clinics, and social service partners.

STEP 05
Warm Handoff

We do more than provide phone numbers. Whenever possible, P.A.V.E coordinates directly with partner providers to help ensure individuals are supported through the next step.

STEP 06
Follow-Up

Follow-up is a key part of our work. We support individuals after the first referral by checking in, helping remove barriers, and encouraging continued engagement.

STEP 07
Stabilization

The long-term goal is to help individuals move toward recovery, housing stability, improved health, and stronger community connection.
